As I said I would when the restructure of the Chain of Command took place.
I, Shroombuck, solemnly and sincerely affirm that, on becoming commissioned within FEDERATION COMMAND, I will with the utmost honour, integrity, and respect, be faithful and bear true allegiance to the Constitution and associated Policies, customs and glorious traditions of FEDERATION COMMAND and all lawful orders of the Officers appointed above me, for as long as I remain an Officer.
I would like to ask that my oath is accepted and I be commissioned once the new Costello Commission for cadre restructuring in the officer base of Federation Command has decided upon my rank.

As a former Rear Admiral, Division Admiral, Fleet Captain, etc... Your return is granted as Captain, however because of the new chain of command be aware that the Rank Revision Panel will have authority on your final rank.
